Use this form for every leave request unless you have an HRIS that captures the same fields. The form is the audit trail; the HRIS is the operational tool.
For very short leave (single day, half day), some teams operate on calendar invites instead — that's fine for ad-hoc time off but not for accrued annual or personal leave.
Honest answers before you download.
- How much notice should I give?
- Per your jurisdiction's award or company policy — typically 2 weeks for annual leave, as soon as practicable for personal/sick leave, longer for parental. The form has the placeholder notice field.
- What about partial-day leave?
- The form supports start-time and end-time fields for partial days. Working-days-requested should reflect the partial-day arithmetic per your award.
- Does this work for parental leave?
- Yes, with the return-to-work note completed. For extended parental leave, pair with a separate return-to-work plan and the parental-leave policy from the HR policies hub.
